Purchase Recommendation Form - Request that the library consider purchasing a new book, video, journal, or database.

Suggestions/Comments/Concerns Form - Do you have comments, suggestions, or complaints concerning JML and the services it offers.

Interlibrary Loan Request Form - Request materials not owned by JML be borrowed for your use.

Book Renewal Request Form - Request renewal, recall, and hold of books and other items you have borrowed.

Item Search Form - Request holds, recalls, and searches of library materials owned by JML.

Library Instruction Request Form - This electronic form allows faculty, staff, or students to request specific library instruction.

Research Consultation Request Form- This electronic form allows faculty, staff, or students to request to schedule a research consultation session.
